    Speaker Bio

    Allyson Y. Schwartz is President and CEO of the Better Medicare Alliance and a former member of the U.S. House of Representatives from Pennsylvania who served from 2005-2015. Better Medicare Alliance is the leading advocacy coalition supporting Medicare Advantage. Having worked as a health service executive, Schwartz was elected to the Pennsylvania State Senate in 1990, serving 14 years until her election to Congress. In the state Senate, Schwartz was the driving force behind Pennsylvania's CHIP program, which was a model for the federal CHIP program five years later. She was the leader in Congress on physician payment reform to encourage value over volume, supported research, innovation, and use of technology to improve quality, efficiency and contain costs. Schwartz is a Distinguished Policy Fellow at the University of Pennsylvania's Leonard Davis Institute of Health Economics, Visiting Fellow at the Penn Wharton Public Policy Initiative and Co-Chair of the Bipartisan Policy Center Health and Housing Task Force.

    Margaret E. O'Kane is the founder and president of the National Committee for Quality Assurance (NCQA).

    She was elected a member of the Institute of Medicine in 1999 and received the 2009 Picker Institute Individual Award for Excellence in the Advancement of Patient-Centered Care. Modern Healthcare magazine has named O'Kane one of the “100 Most Influential People in Healthcare” eleven times, most recently in 2016, and one of the “Top 25 Women in Healthcare” three times. She received the 2012 Gail L. Warden Leadership Excellence Award from the National Center for Healthcare Leadership.

    Speaker Bio

    Erwin Tan is a board-certified internist and geriatrician and is the Director of Thought Leadership ? Health at AARP. Erwin previously served as the director of Senior Corps at the Corporation for National and Community Service. From 2004 to 2010, he served as an assistant professor of medicine at the Johns Hopkins School of Medicine where he was an attending physician in the Division of Geriatric Medicine. He was also a co-investigator in the Baltimore Experience Corps Study. From 2003-2004, Erwin was a White House Fellow serving as a Special Assistant to the Secretary of Veterans Affairs. Before coming to the Washington, D.C. Metropolitan area, Erwin was a faculty member at the University of California, San Francisco School of Medicine, where he served as Geriatric Medicine Fellow and a Primary Care Medicine Resident. Erwin was commissioned as a 2nd Lieutenant in the United States Army Reserves.

    Mary Jane Konstantin, RN, is the Senior Vice President and General Manager of CarePoint for AxisPoint Health, responsible for operational, strategic, and clinically-focused leadership behind delivery of the company’s innovative care management services. Ms. Konstantin has over 25 years of experience with clinically-based solution delivery, utilizing data-driven metrics and innovative technology to achieve successful program outcomes. She has served in operational leadership with numerous health plans and specialty providers, including senior-level roles with Ceridian HCM, NIA/Magellan Health Services, and CareFirst BlueCross BlueShield. Ms. Konstantin holds a Bachelor of Science Degree in Nursing from Vanderbilt University and a Master’s Degree in Health Services Administration from George Washington University.

    Dr. Sandeep Wadhwa is the Chief Health and Medical Officer for Solera Health where is focused on scaling interventions that address the social determinants of health in vulnerable populations. He previously served as the State Medicaid Director of Colorado. He received his undergraduate degree from Wesleyan, medical degree from Cornell and business degree from Wharton. He is board certified in internal medicine and geriatrics.

    Speaker Bio

    Jaan Sidorov is the CEO and President of The Care Centered Collaborative, a Pennsylvania Medical Society organization dedicated to improving patient care through the integration of physician-led medical practices. He is a general internal medicine physician with over 20 years' experience in primary care and population based care coordination. He has also served as a Chief Resident, at Reading Hospital, Reading, Pennsylvania. He received additional training in health services research through the Faculty Development Fellowship of the University of North Carolina at Chapel Hill and in managed care through the American Association of Health Plans certified managed care executive program. Dr. Sidorov is board certified in internal medicine and is a Fellow of the American College of Physicians.
John H. Poelman, MPH

    Speaker Bio

    John Poelman is the Executive Director of the Accountable Care Learning Collaborative. In this role he oversees the execution of the Collaborative’s mission to accelerate the accountable care movement and supplies members with actionable intelligence. Mr. Poelman is also a Senior Director at Leavitt Partners. As Senior Director, Mr. Poelman informs clients on the shifting health care landscape with expertise in accountable care, the implementation of the Affordable Care Act and the corresponding political dynamics. Previously, Mr. Poelman served as a Health Policy Analyst at the U.S. Department of Health and Human Services (HHS). Mr. Poelman earned a master’s degree in public health with an emphasis in health policy from George Washington University and a bachelor’s degree from the University of Utah.

    Speaker Bio

    Raymond Fabius is Co-Founder and President of HealthNEXT. He served as Chief Medical Officer of Truven Health Analytics and as strategic advisor to the President of Walgreens Health & Wellness Division. Dr. Fabius is also the principal of Ab3Health LLC.

    Previous positions include President and Chief Medical Officer for I-trax/CHD Meridian, Global Medical Leader at General Electric and Senior Medical Director for Aetna e.Health Activities. He also served as medical director for CIGNA Health Plan of Pennsylvania, New Jersey and Delaware. Dr. Fabius started his career as a pediatrician ultimately serving as the president of an eight-doctor private pediatric practice.

    Dr. Fabius is a faculty member of the American College of Physician Executives, and the American College of Occupational And Environmental Medicine. Dr. Fabius was a founding member and Vice President for Health Internet Ethics and has played advisory roles within the National Committee for Quality Assurance and the Utilization Review Accreditation Commission.
