PRECONFERENCE I: USING SHARED DECISION MAKING TO DRIVE POPULATION HEALTH STRATEGY

This session will discuss how shared decision making (SDM) can strengthen your population health strategy and drive outcomes. The workshop begins with a brief overview of SDM principles, followed by a panel discussion focused on key outcomes that can be achieved with successful SDM implementation, including: improved patient and provider experience, higher quality of care, and care that aligns with what matters most to patients. We’ll conclude with an inspiring discussion of the key role of culture and leadership as well as a practical overview of important resources, best practices, and strategies proven to facilitate successful implementation of SDM across a range of care delivery settings.
During this half-day workshop, participants will:
- Understand the role of shared decision making in population health.
- Identify examples of health system and payer delivery models that support and facilitate shared decision making.
- Identify ways to implement shared decision making principles across systems of care.

Emily Transue is the Associate Medical Director for the Washington State Health Care Authority, where she leads the Shared Decision Making/Patient Decision Aid certification project. She received her B.S. from Yale University in 1992, M.D. from Dartmouth Medical School in 1996, and Master of Health Administration from the University of Washington in 2013. She is a general internist who practiced for 15 years as a primary care provider before shifting to roles in leadership and management. She has previously worked in multispecialty group practice leadership and Quality Improvement, and as Medical Director for a Medicaid Managed Care Organization. She’s the author of two books, On Call and Patient by Patient, about the experience of practicing medicine. At the Health Care Authority, her role includes working with the Managed Care organizations, leading the Quality Management Team, supporting the Pharmacy team, and supporting the work of Healthier Washington and the Medicaid Transformation Project.

Dr. Goldbach serves as chief medical officer at both Health Dialog and RediClinic, subsidiaries of the Rite Aid Corporation, where he is responsible for the clinical integrity of their various programs, for coordination of their clinical services with client partners, and for providing leadership in the development of new products. Prior to joining Health Dialog and RediClinic, he served as President and CEO of Med-Vantage. Before that he served as a Medical Director for Blue Cross and BlueShield of Massachusetts.
Previously, he also served as CEO, medical director and trustee of a community hospital, president of a physician-hospital organization, and medical director for a rehabilitation hospital, a transitional care unit and a national home care business.
As a board-certified specialist in Pulmonary Disease, he has initiated and directed successful programs in inpatient pulmonary rehabilitation and ventilator management at two chronic care hospitals and also maintained a primary care and pulmonary disease practice.

PRECONFERENCE II: PURSUING EQUITY: THE ROLE OF HEALTH CARE IN ADVANCING EQUITY

Health care has an important role in advancing equity. This session will provide actionable ideas for improving equity by sharing share case studies and “how-to’s” behind the five-pillar framework for health systems to advance equity: (1) making equity a strategic priority, (2) building the structures and processes to support equity, (3) addressing clinical care and other determinants of health that health care can influence (e.g., SES of employees), (4) addressing institutional racism and, and (5) partnering meaningfully with the community. Attendees will have the opportunity to develop next steps to take action that will advance equity in their organizations.

David Ansell, MD, MPH is the Senior Vice President for Community Health Equity at Rush University Medical Center in Chicago. He is an internal medicine physician trained, at Cook County Hospital in the late 1970s, where he spent seventeen years holding a number of positions including Chief of General Medicine/Primary Care. After leaving County, he spent ten years as Chair of the Department of Internal Medicine at Mount Sinai Hospital, located in one of the highest hardship communities in Chicago. He has been particularly involved in health equity work, bringing attention to higher rates of breast cancer mortality for African-American women in Chicago. In 2007 he co-founded the Metropolitan Chicago Breast Cancer Taskforce a not-for-profit that focuses on eliminating the racial disparities in breast cancer mortality.

She implements a theory-driven evaluation approach for IHI’s work in Africa, Europe, Latin America, and North America across a variety of content areas including maternal and child health and safety. Prior to her work at IHI, she was a Research Assistant supporting QI programs at the North Carolina Area Health Education Center (NC AHEC) and with the University of North Carolina Gillings School of Global Public Health on a maternal and child health QI project evaluation. She began her career as a Research Assistant in geriatrics at the Boston Medical Center, and was a 2007-2008 Thomas J Watson Fellow investigating racial identity of multiracial individuals in Brazil, Namibia and South Africa.

Dr. Oehlert has over three decades worth of expertise and wisdom gathered from a life of working in health care. She started her career as a nurse aide at the age of 13 and has served in a variety of clinical, operational, consultative, and strategic roles. Her clinical practice is in Emergency Trauma. She achieved her Doctorate of Nursing in Health Innovation and Leadership. She is a perpetual student of design thinking and culture disruption and currently serves as the Chief Experience Officer for Vidant Health and is adjunct faculty at Eastern Carolina College of Nursing. In her current role, Dr. Oehlert is strategically responsible for the experience and culture at Vidant Health. This includes: patient experience, team member experience, physician experience and burnout, equity and inclusion for patients and team members, and work place safety, as well as areas that contribute to positive culture.

Previously, Ms. Onyekere was a Program Officer at the Robert Wood Johnson Foundation and served as the Executive Director for Health Leads New York.

The Honorable Dr. David J. Shulkin is the ninth Secretary of the U.S. Department of Veterans Affairs. Nominated by President Trump to serve in his Cabinet, Secretary Shulkin was confirmed by the U.S. Senate on February 14th 2017 by a vote of 100-0. Dr. Shulkin previously served as Under Secretary for Health, having been appointed by President Obama and confirmed unanimously by the U.S. Senate in July of 2015.
As Secretary, Dr. Shulkin represents 20 million American veterans and is responsible for the nation’s largest integrated healthcare system, with over 1,200 sites of care serving 9 million Veterans. VA provides essential benefits to veterans including disability payments, educational assistance through the GI Bill, and home loans that have helped millions of American families.
Prior to coming to VA, Dr. Shulkin was a widely respected healthcare executive having served as chief executive of leading hospitals and health systems. He remains a practicing internist, having the distinction of being the first VA Secretary to take care of veterans.

Dr. Andrew Renda is Humana’s Director of Bold Goal (Population Health). His work includes leading strategy, pilots, metrics, research, knowledge management tools and communications related to making progress towards Humana’s Bold Goal of improving community health by 20% by 2020. Previous roles have included advancing clinical models of care through development, implementation and evaluation of population health initiatives aimed at preventing and delaying progression of chronic disease. This included product and benefit design, as well as health projects ranging from messaging campaigns and self-care interventions to clinician-led disease management programs. Significant initiatives include: Chronic Condition Special Needs Plans (C-SNP), Metabolic Syndrome Support Service, Asthma and COLD disease management programs, Sleep Apnea diagnostics and management strategy. He also serves on several Humana clinical and research committees, including: Humana’s Clinical Leadership Network, Clinical Quality Metrics Alignment Government Committee, Comprehensive Health Insights Joint Research Governance Committees, and Humana Publication Committee.

David B. Nash was named the Founding Dean of the Jefferson College of Population Health (JCPH) which caps a 25-year tenure on the faculty of Thomas Jefferson University. He is also the Dr. Raymond C. and Doris N. Grandon Professor of Health Policy and a board-certified internist.
Dr. Nash is a principal faculty member for quality of care programming for the American Association for Physician Leadership and leads their academic joint venture with JCPH. He serves on the NQF Task Force on Improving Population Health and the John M. Eisenberg Award Committee for The Joint Commission. He is also a founding member of the AAMC-IQ Steering Committee. Dr. Nash was recently appointed to the Board of the Pharmaceutical Quality Alliance (PQA) in Washington, DC. Dr. Nash serves on multiple boards and is currently Editor-in-Chief of American Journal of Medical Quality, Population Health Management, P&T, and American Health and Drug Benefits.
